Canon R5 Eye AF not working?

Hi all, my photos aren't sharp, is it me or my equipment? I'm using R5 with 70-200 EF, the attached photo as an example was shot at 1/1000 F4 ISO 3200, case 1 with animal eye tracking and back button focus. You can see the focus point is behind the eye, this is more obvious when my dog is moving. Is it the older lens, the camera or my skills that's causing it to miss focus 50% of the time?
