I must have missed it in the huge number of pages in the manual ( ! ), but what are the settings to lock the exposure and focus spots in the same place in the middle of the screen?

I always seem to manage to get the focus spot away from the centre, and have to manually bring it back.

Am I missing some simple settings?

I don't think you can lock it, but you can always reset it tapping the icon in the corner (circled in red). Just making sure when you say "manually bring it back" you don't mean using the joystick.

You can press the joystick on the R7 which should have the same effect, if I understand the issue correctly.

Only the R3 can lock the spot metering exposure point to the focus point. I think some other brands can do it, but not Canon except the R3. The 1dxII will do it but it's an DSLR.
